On my Lancer, the driver's side outer vent (between the wheel and door), is floppy along the vertical axis. If I tilt it down, vibration or a bump will cause it to jump to pointing up.
Is it hard to get to that vent (IE, removing the entire dash assembly), and can I fix it without getting a new vent insert?

You can pull the vent out without pulling all the dash out. I did mine several years ago but can't remember how. Mitsubishi replaced my vent under warranty but it didn't change.  It's a design fault. 
The only way I suggest you will fix it is pull it apart and some how get more friction between the pins and slots.
